Is there anyone who has gone through CWT6 or type 1 evaluation with FCCPT?
If so, kindly let me know from where can the following deficiencies be fulfilled?
1. History
2. Systems Review
3. Findings that warrant referral
4. Supervision of support staff

Hi,
I had done my Physiotherapy credential verification for New York and was approved in 2004. I also got my AIN (2006), and CGFNS visa screen (expired 2010). I am presently working in Kuwait. Do I have to do my credential verification again if I am planning to work in the US.

Hello Roy,
It would be better to contact the credentialing agency to see how long they are valid. You also might want to contact the NY PT Board to see if your earlier application is still open or would you need to open a new application with the state for Licensing.
As one of our members here had stated recently that as she contacted one of the credentialling agency about the validity of the report for Michigan, she was informed that it would still be valid even after 7 yrs she did it but that whether it is accepted or not will depend on the State.
Do keep in mind that the requirements have changed in most states recently, so they might require a new evaluation report.
And if I remember it right, the applications I had with Michigan as well as Texas states did state that they would keep my file open only for 2 yrs and will be closed if there is any inactivity.
