These are informations collected on some of the Universities offering a PhD in PT or Rehab Science in the US. Since treating patients are part of the program, it should be right to assume that one might need a PT license in that particular state to do a PhD program in PT. For accurate informations one should contact the particular University academic counsellor for details.
The general requirements found to be needed are : application form, transcripts, GRE scores: 900, toefl, letters of recommendation.
1. Miami: intake deadline time: early January. The applicant must meet all the general admissions requirements of the University of Miami Graduate School. Additionally, stipulations include:
· Direct entry into the PhD program is dependent upon having any of the degrees below:
§ An entry-level Doctor of Physical Therapy
§ An advanced Masters Degree with a Bachelors Degree in Physical Therapy
§ A Bachelors Degree in Physical Therapy.
· -An applicant with a Bachelors Degree in Physical Therapy, though able to commence course work in the PHD Program, will be required to complete the credits to equal that of Advanced Masters Degree in Physical Therapy before final admission to the PhD Program.
· Official Transcripts of all college work.
· Three completed recommendation forms with at least one form completed by a physical therapist.
· A letter indicating career goals and objectives.
· Licensure, or eligibility for licensure, as a physical therapist in the State of Florida (must be licensed within 1 year of admission).
· GRE, The University of Miami requires a minimum of 1000 on the verbal and quantitative section.
In addition you are required to take the analytical section.
· Foreign applicants, who complete college and university work in a foreign country, must have all transcripts evaluated by the University of Miami’s Office of International Admissions. http://www.pt.med.miami.edu/x203.xml

2. University of illinois: Offers Phd in Kinesiology, nutrition and rehab and not in PT. Phd in rehab requires atleast 32 hr post Master level education ( effectively a Doctrate level education )
3. University of Utah : Requires Masters or Doctrate in PT, Bachelors degree with high level clinical research experience also considered. requirements:
· The Application for Admission to Graduate School (available online or in hard copy)
· Official transcripts of undergraduate and graduate course work
· For international students, a Test of English as a Foreign Language (TOEFL) score of at least 550, if applicable
· Report of the Graduate Record Exam (verbal, quantitative, and analytical) taken within the past ten years
· The Application for Admission form
· 3-5 letters of recommendation from individuals with knowledge of the applicant’s potential for success in a doctoral program
· Acceptance to the Graduate School at the University of Utah
· A minimum grade point average of 3.0 in all college work and a score of at least 1500 on the Graduate Record Exam

4. University of Kansas: two programs-
[1] Phd in rehab' -requirements: The program is open to persons with a B.S. degree or its equivalent in any of the relevant sciences. Applicants to the program do not have to be physical therapists; however, each candidate is encouraged to have a broad background in biological sciences (including anatomy, physiology, neuroscience, organic chemistry, biochemistry, microbiology, genetics and molecular biology), calculus and statistics. Other admission requirements include:
· A minimum GPA of 3.0 on a 4 point scale in the last 60 credit hours of course work.
· A satisfactory score in the Graduate Record Examination (GRE), obtained within the previous five years.
· A satisfactory score in the Test of English as a Foreign Language (TOEFL), obtained within the previous two years (for international applicants only).
· Three letters of reference from persons familiar with the applicant's professional and/or academic abilities.
· A curriculum vitae detailing work history, formal education, continuing education, licensing and certification, professional organizations, honors and awards, publications, presentations, grants, etc.
· Transcripts from all colleges attended.
· A written educational plan describing the applicant's goal(s) and objective(s).

5. Texas womens university : Post-professional Ph.D. in Physical Therapy. Admission Requirements General Admission Requirements
§ hold an earned baccalaureate degree or equivalent from a U. S. college or university accredited by a regional or general accrediting agency, or an equivalent degree from a foreign college or university. The baccalaureate degree must be substantially equivalent to a baccalaureate degree granted by TWU.
§ Have a minimum 3.0 grade point average (GPA) for the last 60 hours of undergraduate study and a minimum 3.0 GPA for all prior graduate work.
§ Present an official transcript from each university or college attended with degree earned indicated on the transcript.
§ Submit admission application for Graduate Admission with appropriate application fee.
§ United States citizens, Permanent Residents, and International Students - complete online application via Texas Common Application.
§ International students also must submit a minimum score of 550 on the TOEFL (Test of English as a Foreign Language) or a minimum score of 213 on the computer-generated version of the TOEFL. Also required is a minimum score of 55 on the TSE and 5 on the TWE. For more info, please see international student requirements.
§ Satisfy any additional requirements of an individual academic component for admission to a specific graduate program offered by that component.
§ All transcripts of college or university work completed in foreign countries must be sent to Office of International Education on the Denton campus. Please call (940) 898-3048 for more information. All coursework from a foreign university must be evaluated by TWU's International Education Office to establish U.S. equivalencies.
Additional Requirements for the PhD Program in Physical Therapy: In addition to the general requirements for admission to the Graduate School, prospective doctoral students also must meet the following specific requirements of the School of Physical Therapy:
§ Graduation from an approved (CAPTE accredited or equivalent) entry-level physical therapy program.
§ An earned master's degree or equivalent from a U. S. college or university that is accredited by a regional or general accrediting agency, or an equivalent degree from a foreign college or university. A master’s degree in physical therapy is not required; the degree may be in a related area.
§ A license to practice physical therapy in the United States or a minimum of 24 months of full time clinical practice (beyond that required for entry-level education) within the last 3 years.
§ A basic course in statistics within the last 10 years.
§ A minimum 3.0 grade point average (GPA) on a 4.0 system.
§ Two letters of recommendation, preferably one from an employer and one from the last school attended.
§ Written statement indicating the current focus of experience, educational objectives, and an evaluation of personal strengths and weakness.
§ Resume or Curriculum Vitae.

6. University of Iowa: Phd in Rehab science. Application deadlines:
a. Fall semester—May 15
b. Spring semester—October 15
c. Summer session—March 15
Materials to be sent to the department:
§ Your official academic records/transcripts
§ Three (3) Letters of Recommendation*
§ A Statement of Purpose*
§ A second set of your official academic records/transcripts
§ Your official GRE General Test scores from the Educational Testing Service (the University's institutional code is 6681)
§ International students must send their official TOEFL scores from Educational Testing Service (the University's institutional code is 6681).

7. Universtiy of central Arkansas: Students wishing to pursue the Doctor of Philosophy (PhD) degree program in physical therapy will be admitted through a process involving a mutual decision between the graduate dean and the Department of Physical Therapy. Applicants must submit the application materials to the Graduate School and should contact the department for further information. The following materials must be submitted:
§ A completed application for admission to the Ph. D. program from the Department of Physical Therapy (online application and resources at [link]
§ An application for admission to the Graduate School at the University of Central Arkansas (online application and resources at http://www.uca.edu/graduateschool/admission_process/)
§ Scores of the Graduate Record Examination (GRE). A minimum of 1000 is required on the general test and a minimum of 3.5 is required on the analytical writing test (http://www.uca.edu/divisions/academic/graduate)
§ Score of the Test of English as a Foreign Language (TOEFL) is also needed for international students. Contact the Graduate Office for the minimum acceptable score.
§ Two letters of reference
§ A brief letter indicating a statement of purpose
